Book your Covid - 19 Booster Vaccine

Please do not turn up at the vaccine centers unless you have an appointment booked

You can book your COVID-19 booster vaccine dose online if you have been contacted by the NHS and you are either:

  • aged 50 and over
  • aged 16 and over with a health condition that puts you at high risk from COVID-19
  • a frontline health or social care worker

You'll only be able to book an appointment for a booster dose if it's been at least 6 months (180 days) since your 2nd dose of the vaccine.

Type 1 opt-out Data Sharing

This link from NHS Digital explains why data is shared, it contains their transparency & privacy notices for doing so and also points to links where the patient can opt out if they want to:

Data sharing with NHS Digital will start on 1 September 2021.

If you wish to register a Type 1 Opt-out with your GP practice before data sharing starts with NHS Digital, this should be done by returning this form to your GP practice by 23 June 2021 to allow time for processing it. If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can return the form by post to Brownhill Surgery, 790 Whalley New Road, Blackburn, BB1 9BA or by email to brownhill.surgery@nhs.net.  Alternatively, you could call NHS Digital on 0300 303 5678 for a form to be sent out to you. 

Paper copies of this form will also be available at reception for those who do not have access to online services.
