Patient Access through the NHS App
The NHS App allows you to book/cancel your own appointments online, order your repeat prescriptions, update your own address details as well as access some parts of your medical record. Please do not send messages through the online access system as this is not checked daily and therefore important information could be missed. If you have any questions, please contact the reception team.

The ‘Patient Access’ registration form can be obtained either by clicking on the link below or from reception.
Once you have completed the form, please hand it to reception with your two types of identification (one being photographic). A personal user ID number will then be generated. This will only take a few minutes.
With the NHS App, you can book and cancel your GP appointments, receive appointment reminders and add the details to your calendar. You can monitor the medication you're taking and share information with healthcare professionals.
The app also makes it easier to care for your loved ones. You can add children and elderly relatives to the app and arrange their appointments. Children between the ages of 12 and 15 cannot be added by a parent or carer without permission.
There are tools that let you track your weight and blood pressure, and help you take better control of your health and wellbeing.
If you would like to sign up to use this service, you MUST first complete a simple registration form, as a personal user ID number is required before you can use this facility. Please note, we ask that you provide us with two forms of identification, one of which MUST be photographic e.g passport, driving licence etc. before any registration can be authorised. If you are already registered with patient access, simply log in to your account, go into 'my account' and click on 'my practice'. This will bring up the details you will need to complete your registration with MyGP.
It is your responsibility to keep your user ID number secure.
